Many thanks to Charles for all his hard work. Interesting article on med reconc. One thing...there isn't a place on our med reconc. to put name of pt's pharmacy on admit is there?
My record for meds I had to enter in med. rec. is 26! Also, one thing to be alert for on the ER med reconc., is the dose and number of tabs. Insurance companies are on board w/patients doing half tabs of meds to save $. We had a pt. recently in overflow w/Digoxin 0.25 mg., l/2 tab listed on ER med. reconc. Neither pharmacy nor the ER nurse saw the l/2 tab so the pt. got 1 tab in ER. MM
